Some females are not able to bear a child as they experience fertility complications like PCOD (Polycystic Ovarian Disease), cyst, fibroids growth, blocked fallopian tubes, endometriosis, dermoid, scar tissues development in the pelvis (pelvic adhesions), and uterine anomalies like uterine adhesions (such as Asherman’s syndrome which results in scar tissues inside the uterus) and uterine septum. These can lead to disturbances in the normal functioning of the female reproductive system, thereby reducing the chances of women getting pregnant naturally.
Dr. Chaitali, a renowned gynaecologist in Mumbai, Juhu at Nanavati Superspeciality Hospital can help manage and treat such conditions by offering laparoscopy and hysteroscopy surgeries to improve female fertility, collectively referred to as fertility enhancing surgeries. Different surgical treatment options for managing infertility issues offered at Nanavati Superspeciality Hospital, best fertility treatment hospital in Mumbai, Juhu are as follows :
Laparoscopy surgery in Mumbai
It is an advanced, minimally-invasive procedure to diagnose and fix various infertility complications. It is a 30-90 minutes procedure that is carried out by first providing the patient with general anesthesia (or sometimes spinal anesthesia). Next, a hollow needle is inserted through which nitrous oxide or carbon dioxide gas is released slowly into the abdomen to inflate it and lift away the abdominal wall from the internal organs for getting a better inside view.
Then, a small incision is made in the belly through which a thin tube fitted with a camera and light at one end (a laparoscope) is inserted. It helps in clearly visualizing the detailed unusual changes that occur inside the abdominal or pelvic organs. To assist some surgeries, a laser device may be connected to the laparoscope. Sometimes, a sample of affected tissue may be collected through laparoscopy technique and sent to a laboratory for a biopsy.
After completion of the surgery, the inflated belly is returned to its normal state by releasing the filled gases and the incision is sutured and covered with a surgical dressing or bandage. The scars from this surgery are small and over time they disappear.
Diagnostics- Laparoscopy can be performed to check for any conditions that hamper the natural conception for the women. This could be related to fibroids, adhesions, cysts, or infection. It is conducted after doing the initial fertility tests which do not reveal the specific underlying cause of infertility.
Therapeutics- It can be conducted for removal of fibroids, cysts, or adhesions; drilling in the ovary (for PCOS patients), drainage of fluid-filled inside the tubes, tubal reconstructions, and occlusion of tubes (in case of hydrosalpinx). Hence, the different types of fertility-enhancing surgeries that can be performed by this approach include:
Myomectomy (Fibroidectomy)- to remove large or multiple uterine fibroids (non-cancerous growths inside the uterus), also called leiomyomas that develop during the childbearing years of the women
Recovery
Soon after the surgery, the patient is shifted to the recovery room to monitor her vitals for 2-4 hours. From the very next day, the patient can resume her normal daily activities but strenuous activities must be avoided for at least a week. For the first few days, it is normal to expect slight bruising around the incision and some bloating.
It is a diagnostic and therapeutic procedure that makes use of a special viewing tool called hysteroscope fitted with light and camera on the top to get an insight of the uterine lining and cavity on the video screen. This is a short outpatient procedure that is done using sedatives or anesthesia. Before examining the shape of the uterine cavity and lining with a hysteroscopy and figuring out the possible abnormal changes, the patient needs to come with an empty urinary bladder.
To visualize the inside of the vagina and cervix, the doctor firsts inserts a speculum (lubricated tool) into the vagina to separate apart the vaginal walls. Next, the tip of the hysteroscope is placed at the vaginal opening and gently moved inside through the cervix to reach the uterus. For a better, clear view of the uterus, a gas or liquid may be filled into the uterus through the hysteroscope. A dye is also passed via the cervix to check for patency of fallopian tubes. As per the indication, hysteroscopy and laparoscopy can be performed together.
Hysteroscopy can diagnose and treat various fertility problems in women. Therefore, doctors conduct it to:
Recovery
After the hysteroscopy procedure, the patient is sent to the recovery room and monitored by the doctor for 1-4 hours. Mostly, the patient can return home after daycare, and rarely she might be asked to stay back overnight at the facility. For the first 1-2 days after hysteroscopy, the patient can expect some vaginal bleeding. Some patients report experiencing stomach sickness, slight dizziness, or mild pain in the belly. All of this discomfort can resolve within 24 hours.
